Development of the MCM-D Technique for Pixel Detector Modules

Dünnfilmtechnologie, die MCM-D Technik, und Ihre Anwendung zum
Aufbau von hybriden Pixeldetektor Modulen.
Das ATLAS Experiment am LHC wird über ein
Pixeldetektorsystem verfügen. Die kleinste mechanische Einheit
des Pixeldetektors sind Multichipmodule. Die wichtigsten
Komponenten dieser Module sind 16 Elektronikchips, ein
Kontrollchip und ein Sensor, der über mehr als 46000
Pixelzellen verfügt. MCM-D ist eine verbesserte
Technologie um das notwendigen Signalbussystem und das
Stromversorgungssystem direkt auf den Sensor aufzubauen.
In Zusammenarbeit mit dem Fraunhofer Institut für
Zuverlässigkeit und Mikrointegration, IZM, wurde der
Dünnfilmprozess überprüft und weiterentwickelt. Das
Vielschichtsystem wurde entworfen und sowohl für das
Verbindungssystem als auch für die mehr als 46000
Pixelkontakte optimiert.
Labormessungen an Prototypen haben gezeigt, dass ein
komplexes Verbindungsschema für geometrieoptimierte
Einzelchips durchführbar ist und
vernachlässigbaren Einfluss auf die Leistungsfähigkeit der
Auslesechips hat. Ein vollständiges Modul wurde gebaut; und es
wurde nachgewiesen, dass sich die Technologie eignet um
Pixeldetektormodule zu bauen. Weitere Tests beinhalten u.a. die
Untersuchung des Einflusses von hadronischer Bestrahlung auf die
Dünnfilmlagen. Einzelchipaufbauten wurden auch in einer
Teststrahlumgebung betrieben und die Umsetzbarkeit der
Sensoroptimierung konnte gezeigt werden. Es wird ein Überblick
über das Potential und die Perspektive der MCM-D Technologie
in zukünftigen Experimenten gegeben.This thesis treats a copper--polymer based thin film technology,
the MCM-D technique and its application when building hybrid
pixel detector modules.
The ATLAS experiment at the LHC will be equipped
with a pixel detector system. The basic mechanical units of the
pixel detector are multi chip modules. The main components of
these modules are: 16 electronic chips, a controller chip and a
large sensor tile, featuring more than 46000 sensor cells.
MCM-D is a superior technique to build the necessary signal bus
system and the power distribution system directly on the active
sensor tile.
In collaboration with the Fraunhofer Institute for Reliability and
Microintegration, IZM, the thin film process is reviewed
and enhanced. The multi layer system was designed and optimized
for the interconnection system as well as for the 46000 pixel
contacts. Laboratory measurements on prototypes
prove that complex routing schemes for geometrically optimized single
chips are suitable and have negligible influence on the front--end
chips performance. A full scale MCM-D module has been built and
it is shown that the technology is suitable to build pixel
detector modules. Further tests include the investigation of the
impact of hadronic irradiation on the thin film layers. Single
chip assemblies have been operated in a test beam environment and
the feasibility of the optimization of the sensors could be shown.
A review on the potential as well as the perspective for the
MCM-D technique in future experiments is given
ACCEPT®: A Complementary Anthroposophical Program for the Palliative Treatment of Lung Cancer – Rationale and a Randomized Feasibility Study
Background: Lung cancer is the oncological disease with the highest mortality worldwide. Health-related quality of life is severely compromised in the majority of patients. While the efficacy of early palliative psychosocial therapy has been demonstrated in several recent studies, appropriate therapy modules could so far not be integrated into daily practice of care. Therefore, an additive multimodal treatment concept for oncological centers was drafted: the Additive anthroposophic integrative medicine Cancer Concept of Early supportive or Palliative lung cancer Treatment (ACCEPT®). Patients and Methods: The first module consisted of a 3-month health education program, the second module was a concept of psychosocial interventions, and the third module was a supervised home training program. Between 2017 and 2018, 20 lung cancer patients (UICC IIIB/IV) were included and randomly assigned to treatment (n = 10) or a waiting control group (n = 10). The treatment group started ACCEPT® for 3 months immediately after diagnosis and received also standard oncological care (SOC) while the waiting control group received SOC only for 3 months, followed by ACCEPT® after this period. Health-related quality of life, disease management, disease-specific symptoms, and feasibility of the ACCEPT® were monitored at 4 time points. Results: 7 out of 10 patients in the treatment group (3 dropped out) and 6 out of 10 in the waiting control group (4 died during the intervention) completed treatment. Discussion/Conclusion: Lung cancer patients with high symptom load may benefit from ACCEPT®. The feasibility of this adjunctive therapy was demonstrated. The combination of SOC and ACCEPT® is feasible and applicable to a heterogeneous patient group and should be further evaluated with respect to efficacy and dosing.Hintergrund: Lungenkrebs ist die onkologische Erkrankung mit der weltweit höchsten Mortalität. Die gesundheitsbezogene Lebensqualität ist bei der Mehrzahl der Patienten stark beeinträchtigt. Obwohl die Wirksamkeit der frühpalliativen psychosozialen Therapie in mehreren neueren Studien nachgewiesen wurde, konnten entsprechende Therapiemodule bisher nicht in die tägliche Versorgungspraxis integriert werden. Daher wurde ein additives multimodales Behandlungskonzept für onkologische Zentren entworfen: additives anthroposophisch-integrativ medizinisches Krebskonzept der frühen unterstützenden oder palliativen Lungenkrebsbehandlung (ACCEPT®). Patienten und Methoden: Das erste Modul bestand aus einer dreimonatigen onkologischen Gesundheitsschulung, das zweite Modul war ein Konzept für psychosoziale Interventionen, und das dritte Modul war primär ein Training im häuslichen Umfeld. Zwischen 2017 und 2018 wurden 20 Lungenkrebs-Patienten (UICC IIIB/IV) eingeschlossen und nach dem Zufallsprinzip der Behandlung (n = 10) oder einer wartenden Kontrollgruppe (n = 10) zugeteilt. Die Behandlungsgruppe begann unmittelbar nach der Diagnose für 3 Monate mit ACCEPT® und erhielt auch die onkologische Standardbehandlung, während die wartende Kontrollgruppe für 3 Monate nur onkologische Standardbehandlung erhielt, gefolgt von ACCEPT® nach dieser Zeit. Die Lebensqualität, das Krankheitsmanagement, die krankheitsspezifischen Symptome und die Durchführbarkeit von ACCEPT® wurden zu 4 Zeitpunkten beobachtet. Ergebnisse: 7 von 10 Patienten in der Behandlungsgruppe (3 schieden aus) und 6 von 10 Patienten in der wartenden Kontrollgruppe (4 starben während der Intervention) beendeten die Behandlung. Diskussion/Schlussfolgerung: Lungenkrebs-Patienten mit hoher Symptombelastung können von ACCEPT® profitieren. Die Durchführbarkeit dieser Begleittherapie wurde nachgewiesen. Die Kombination von onkologischer Standardbehandlung und ACCEPT® ist durchführbar und auf eine heterogene Patientengruppe anwendbar und sollte im Hinblick auf Wirksamkeit und Dosierung weiter evaluiert werden
KRASG12C/TP53 co-mutations identify long-term responders to first line palliative treatment with pembrolizumab monotherapy in PD-L1 high (≥50%) lung adenocarcinoma
Background: Pembrolizumab is a standard of care as first line palliative therapy in PD-L1 overexpressing (≥50%) non-small cell lung cancer (NSCLC). This study aimed at the identification of KRAS and TP53-defined mutational subgroups in the PD-L1 high population to distinguish long-term responders from those with limited benefit.
Methods: In this retrospective, observational study, patients from 4 certified lung cancer centers in Berlin, Germany, having received pembrolizumab monotherapy as first line palliative treatment for lung adenocarcinoma (LuAD) from 2017 to 2018, with PD-L1 expression status and targeted NGS data available, were evaluated.
Results: A total of 119 patients were included. Rates for KRAS, TP53 and combined mutations were 52.1%, 47.1% and 21.9%, respectively, with no association given between KRAS and TP53 mutations (P=0.24). By trend, PD-L1 expression was higher in KRAS-positive patients (75% vs. 65%, P=0.13). Objective response rate (ORR), median progression-free survival (PFS) and overall survival (OS) in the KRASG12C group (n=32, 51.6%) were 63.3%, 19.8 months (mo.) and not estimable (NE), respectively. Results in KRASother and wild type patients were similar and by far lower (42.7%, P=0.06; 6.2 mo., P<0.001; 23.4 mo., P=0.08). TP53 mutations alone had no impact on response and survival. However, KRASG12C/TP53 co-mutations (n=12) defined a subset of long-term responders (ORR 100.0%, PFS 33.3 mo., OS NE). In contrast, patients with KRASother/TP53 mutations showed a dismal prognosis (ORR 27.3%, P=0.002; PFS 3.9 mo., P=0.001, OS 9.7 mo., P=0.02).
Conclusions: A comprehensive assessment of KRAS subtypes and TP53 mutations allows a highly relevant prognostic differentiation of patients with metastatic, PD-L1 high LuAD treated upfront with pembrolizumab
Genetic and geographic influence on phenotypic variation in European sarcoidosis patients
IntroductionSarcoidosis is a highly variable disease in terms of organ involvement, type of onset and course. Associations of genetic polymorphisms with sarcoidosis phenotypes have been observed and suggest genetic signatures.MethodsAfter obtaining a positive vote of the competent ethics committee we genotyped 1909 patients of the deeply phenotyped Genetic-Phenotype Relationship in Sarcoidosis (GenPhenReSa) cohort of 31 European centers in 12 countries with 116 potentially disease-relevant single-nucleotide polymorphisms (SNPs). Using a meta-analysis, we investigated the association of relevant phenotypes (acute vs. sub-acute onset, phenotypes of organ involvement, specific organ involvements, and specific symptoms) with genetic markers. Subgroups were built on the basis of geographical, clinical and hospital provision considerations.ResultsIn the meta-analysis of the full cohort, there was no significant genetic association with any considered phenotype after correcting for multiple testing. In the largest sub-cohort (Serbia), we confirmed the known association of acute onset with TNF and reported a new association of acute onset an HLA polymorphism. Multi-locus models with sets of three SNPs in different genes showed strong associations with the acute onset phenotype in Serbia and Lublin (Poland) demonstrating potential region-specific genetic links with clinical features, including recently described phenotypes of organ involvement.DiscussionThe observed associations between genetic variants and sarcoidosis phenotypes in subgroups suggest that gene–environment-interactions may influence the clinical phenotype. In addition, we show that two different sets of genetic variants are permissive for the same phenotype of acute disease only in two geographic subcohorts pointing to interactions of genetic signatures with different local environmental factors. Our results represent an important step towards understanding the genetic architecture of sarcoidosis
Randomised open phase II study for the examination of the tolerance, safety and effectiveness of Viscum album extract in the palliative, additive treatment of advanced non-small cell bronchial carcinoma
Das Bronchialkarzinom ist weltweit als eine der häufigsten malignen Neoplasien
und mit der höchsten krebsbedingten Mortalität assoziiert. Die größte
Subgruppe wird unter dem Begriff „nichtkleinzellige Bronchialkarziome“ (NSCLC)
zusammengefasst. Die Therapie des fortge¬schrittenen NSCLCs konnte in den
letzten Jahren verbessert werden, ist aber – vor dem Hinter¬grund einer 5
-Jahres-Überlebensquote um 1 % – weiterhin unbefriedigend. Misteltherapie
(adjuvante subcutane Therapie mit Viscum album l.) beim NSCLC ist in
Kasuis¬tiken und 6 klinischen Studien – davon 5 randomisierte, kontrollierte
Studien – beschrieben. Die Evidenz dieser Therapie ist noch nicht ausreichend
und steht einer – zumindest in Deutschland, der Schweiz und Österreich –
breiten Anwendung entgegen. In einer offenen, randomisierten Phase-II-Studie
mit n = 50 Patienten wurde die Wirkung von Viscum album l. (Iscador QuS) im
Verlauf der Chemotherapie beim NSCLC (Stadium IIIb / IV) zu ausgewählten
Zeitpunkten untersucht. Als primärer Endpunkt der Untersuchung wurde das
mediane Überleben gemessen, als sekundäre Endpunkte wurden die Remissionsrate,
das progres¬sionsfreie Intervall, die Lebensqualität sowie die Verträglichkeit
der Chemotherapie überprüft. ERGEBNISSE Von 50 randomisierten Patienten
konnten die Daten von 49 Patienten ausgewertet werden, ein Patient hatte keine
Studienmedikation erhalten. Die mediane Überlebenszeit wurde lediglich in der
Kontrollgruppe erreicht, in der Therapiegruppe (Chemo- plus Misteltherapie)
verstarben während des Beobachtungszeitraums weniger als 50% der Patienten.
Das quartile Überleben in der Therapiegruppe betrug 259 Tage, in der
Kontrollgruppe (alleinige Chemotherapie) 170 Tage, die Differenz lag bei 89
Tagen zugunsten der Therapiegruppe. Im Log-Rank-Test ergab sich damit noch
kein signifikanter Unterschied zwischen den Überlebenszeitkurven (p = 0,83).
Bezüglich der Remissionsrate war keine statistische Berechnung möglich, da bei
keinem Patien¬ten eine komplette Remission erreicht wurde. Das
progressionsfreie Intervall lag in der Therapiegruppe bei 272 Tagen, in der
Kontrollgruppe bei 190 Tagen. Im Log-Rank-Test ergab sich kein signifikanter
Unterschied zwischen beiden Gruppen (p = 0,28). Die Lebensqualität, gemessen
mit dem EORTC-C30- und dem EORTC-LC13-Bogen, ergab im Gruppenvergleich keine
signifikanten Ergebnisse zugunsten der Therapiegruppe. Im Score für den
Global-Health-Status ergaben sich für die Therapiegruppe / Kontrollgruppe die
Mittelwerte: 55,99 / 55,17 (p = 0,64). Chemotherapie bedingte schwere
unerwünschte Ereignisse waren in der Therapie¬gruppe seltener Chemotherapie-
bedingt als in der Kontrollgruppe (6/37 vs. 17/37, p = 0,034). DISKUSSION Die
Ergebnisse zeigen bezüglich des Hauptzielparameters Überleben keine
signifikante Diffe¬renz im Vergleich von Therapie- und Kontrollgruppe. Die
Kaplan-Meyer-Kurve weist jedoch eine Kreuzung der Kurven zugunsten der
Therapiegruppe auf, was auf einen verzögert eintreten¬den positiven Effekt der
palliativen-additiven Therapie mit Viscum album l. hindeuten könnte. Eine
begrenzte Aussagekraft der Studiendaten ergibt sich aufgrund der
monozentrischen Studien¬durchführung, der Durchführung in einem Haus mit
Spezialisierung auf Misteltherapie (Beob¬achterbias, Patientenbias), der
kleinen Fallzahl sowie der in beiden Therapiearmen eingetretenen längeren
Überlebenszeit, die um 40 % über der ursprünglich angenommenen Planung lag.
Dementsprechend war die Studie underpowered, hier kann davon ausgegangen
werden, dass dies einen erheblichen Einfluss auf die Ergebnisse bezüglich des
Hauptzielparameters gehabt hatte. Ein besonderer Sachverhalt ist durch das
frühe Versterben von 20 % der Patienten (5 / 25) der Therapiegruppe in den
ersten 50 Behandlungstagen seit dem Screening gegenüber keinem ver¬storbenen
Patienten in der Kontrollgruppe gegeben. Trotz der randomisierten
Gruppenzuteilung gibt es Anhaltspunkte dafür, dass im Therapiearm eine Häufung
besonders kritisch erkrankter Patienten entstanden war – dies führte bei der
kleinen Patientenzahl möglicherweise zu einer Verzerrung der Ergebnisse
zuungunsten des Therapiearms. Ein Zusammenhang zwischen hoher Mortalität und
Induktionsphase der Misteltherapie erscheint eher unwahrscheinlich.
SCHLUSSFOLGERUNGEN Die durchgeführte Studie kann keine signifikante
Überlegenheit der Therapie mit Viscum album l. bei gleichzeitiger
Chemotherapie mit Cisplatin / Docetaxel gegenüber einer alleinigen
Chemo¬therapie mit Cisplatin / Docetaxel zeigen. Ein Kreuzen der
Überlebenszeitkurven im Gruppen¬vergleich zugunsten der Therapiegruppe mit
Viscum album bietet eine Grundlage für weiter¬gehende Forschung. Eine
Zielsetzung der Phönix-1-Studie lag in einer Abschätzung der Wirksamkeit der
adjuvanten Behandlung eines fortgeschrittenen Bronchialkarzinoms. Hier konnten
Hinweise sowohl zum Überleben als auch zur Verträglichkeit der Chemotherapie
zugunsten des Therapiearms feststellt werden. Vor dem Hintergrund der
monozentrischen Studienkonzeption und der sehr kleinen Fallzahl werden diese
Hinweise im Sinne eines Pilotprojektes interpretiert. Damit ermöglichen die
vorgelegten Ergebnisse eine differenzierte Planung weiterer Projekte.

MisteltherapieIn an open, randomised phase II study with n = 50 patients the effectiveness
of Viscum album 1 (Iscador QuS) was examined over the course of chemotherapy
for NSCLC (stage IIIb/IV) at selected points in time. Median survival was
measured as the primary endpoint of the examination, secondary endpoints were
the remission rate, progression-free interval, quality of life as well as the
tolerance of chemotherapy. Results: Out of 50 randomised patients we were able
to evaluate the data of 49 patients. In the therapy group (chemo- plus
mistletoe therapy) the quartile survival was 259 days, in the control group
(chemotherapy only) it was 170 days, with a difference of 89 days in favour of
the therapy group (p = 0.83). Quality of life, measured with the EORTC-C30
questionnaire and the EORTC-LC13 questionnaire. Discussion: With regards to
the main outcome measure, survival, the results show no significant
differences in a comparison between the therapy group and the study group.
However, the Kaplan-Meier curve shows a crossing of the curves in favour of
the therapy group, which could point towards a positive effect of adjuvant
therapy with Viscum album 1. Conclusions: The study conducted cannot document
a significant superiority of therapy with Viscum album 1 concomitant to
chemotherapy with Cisplatin/Docetaxel over chemotherapy with
Cisplatin/Docetaxel on its own. Crossing of the survival curves in a group
comparison in favour of the therapy group treated with Viscum album offers a
basis for further research
